Gallbladder Problems After Weight Loss Surgery

Gallbladder problems are extremely common in obese and overweight individuals. These problems within the gallbladder are called Gallstones, which are caused primarily by:

Why Gallstones Form After Bariatric Surgery

According to the other research, a greater prevalence of gallstones has been reported in gastric sleeve surgery patients compared to the overall obese population. Cholelithiasis, or gallstones, occurred in 30% of patients receiving gastric sleeve surgery.

However, the present theories posit the operative dissection of the vagal back influences rectal contractility and a non-physiological reshaping of their gastrointestinal tract; for example, duodenal exception contributes to enhanced gallstone formation. The current study assessed the prevalence of gallstone formation following gastrectomy for stomach cancer and identified as risk factors for gallstones.

Why Test Your Pouch?

Weight loss surgery is a powerful tool, not a solution. As time passes, it is not uncommon for people to fall back into old habits that are not beneficial to weight loss and may even stretch the pouch, making it even harder to lose weight. The pouch test is for you if you feel that your pouch, or stomach, is not as effective in restricting food intake as it should.

This may occur if you have hit a stall or even started to gain weight back after weight loss surgery, especially gastric bypass or gastric sleeve surgery. You may also need to test your pouch if you have noticed that your portion sizes have gotten noticeably larger and you are not feeling as much restriction as you once felt.

Signs That Your Pouch Has Stretched
  • Portions have gotten larger
  • You don’t feel as much restriction with food intake
  • You have gained weight or hit a long-term stall
  • You are consuming more than 9 ounces during a meal
How Did I Stretch My Pouch?

It is normal for there to be a slight increase in pouch size or volume over time. Pouches average around 5 ounces in size at one year post-op and typically enlarge to 6 ounces in the second year post-op. Pouch sizes can also vary quite a bit. At one year post-op, a pouch size can vary from about 2.5 ounces to 9 ounces.

Pouch stretching is natural and may or may not reflect you doing something wrong. Your pouch does not stretch by one unhealthy meal, nor does it stretch by one single large meal. You can, however, stretch your pouch by eating just a small amount of excess food over and over again.

Tips to Avoid Your Pouch from Stretching:

Be mindful of your body while you eat. Pay attention to cues of hunger and fullness without feeling uncomfortable, starving, or overly full. Figure out how much your pouch can hold using this pouch test, then make that your maximum meal size moving forward. For best results, keep meals to 6 ounces or less, even if you know your pouch can hold more. Measure your portions to be sure. It’s best not to guess!

How to Test Your Pouch

Fast (no food or drink) 60 minutes before the test. Test your pouch during your first meal of the day.

  1. Measure (by weight) 8 ounces of cottage cheese or mashed potatoes on a food scale.  If you are doing this on a plate or bowl make sure to tare out the weight of the bowl so it is not included in the 8 ounces.
  2. Eat as much as you can to feel full (when you feel like one more bite could cause nausea or pain)
  3. Measure by weight the remaining mashed potatoes or cottage cheese that you could not eat (leftovers).
  4. Subtract that number from 8 (8 ounces that you started with) to get the number of ounces your pouch can hold.

Repeat these steps every 3 months if you want to see how your pouch size increases in time.

What to do if Your Pouch Stretched

If you have tested your pouch and you can hold more than 8 ounces or you just need help getting back on track with your diet, we recommend you start the 10 Day Pouch Reset Diet or the 5 Day Pouch Reset Diet. This will take you through the transition process as you did right after surgery and will lead you to a healthy diet that you can continue long term.

Key Tips for Long-Term Success:

  • Eat solid food at meal times
  • Avoid liquid with meals, 30 minutes before and 30 minutes after
  • Eat a minimum of 60 grams protein/day
  • Eat until you feel full but not uncomfortable or nauseous.
  • Measure out 6 ounces of food maximum, even if you feel you can eat more.

What Are Gallstones?

Gallstones are one of the most frequent and costly diseases, known as cholelithiasis. Gallstones are hard, rigid particles found in the gallbladder. The gallbladder is an organ within the human body. It is located in the abdominal cavity on the right side of the body. The gallstones are formed by the chemical reactions of digestive fluid.

The primary function of the gallbladder is the storage of bile.  Most types of gallstones have little to no symptoms, but in the case of an infection, gallstones can lead to severe health complications. This critical condition can become life-threatening.

Gallstones are extremely common, affecting 10 to 15% of Americans (nearly 25 million individuals).

Clinical Signs and Symptoms

The clinical signs and symptoms of gallstones depend upon various factors. The signs and symptoms are separated into two categories: prominent or asymptomatic. People are usually unaware of gallstones in the gallbladder, and it typically reflects the asymptomatic condition.

  • Pain in Abdominal Cavity

In the symptomatic state, people have severe painful experiences. They experience intense pain in their abdominal cavity. This pain may prolong form several minutes to several hours. This painful experience may turn into a sudden headache which is followed by nausea, vomiting, and digestive problems.

  • Size

The size of gallstone depends upon the clinical health condition and age of the patient. Generally, the size of gallstone ranges from a small particle to a wheat grain or kidney bean. Several factors depend upon the growth of gallstones. It has been noted that in certain individuals the gallstones have shown rapid progression as compared to other individuals.

  • Causes

There are various causes of gallstones such as if the digestive fluid contains a high concentration of cholesterol. As a clinical condition, the liver secretes elevated amount of digestive fluid which is rich in cholesterol and the digestive fluid, bile is not capable of dissolving it.

Furthermore, instead of cholesterol, the digestive fluid contains a high concentration of bilirubin which may be an alarming sign of various health complications such as liver cirrhosis, jaundice, and biliary tract infections.

Types of Gallstones
  • Cholesterol Gallstones

The cholesterol gallstone is one of the most common types of gallstone found in the gallbladder. The appearance of cholesterol gallstone is yellowish in color.  These types of gallstones are mainly composed of non-dissolved cholesterol in its chemical composition.

  • Pigment Gallstones

The pigment gallstone is the second common type of gallstone formed in the gallbladder. The physical appearance of pigment gallstone is dark brown in color. In several cases when patients have elevated amounts of bilirubin, the gallstones are black in color.

5 Risk Factors for Cholelithiasis

The 5-F rule is used to determine the 5 risk factors that lead to the development of cholelithiasis in the event of upper abdominal pain. Particular groups of individuals have a higher chance of developing the disease over other individuals.1

  • Fair: more prevalent in the Caucasian population
  • Fat: BMI>30
  • Female
  • Fertile: one or more children
  • Forty: age ≥40
Who is More Prone to Gallstones
  • Girls who have additional estrogen in the body because of pregnancy, hormone replacement therapy, or birth control pills might be more inclined to produce symptoms.
  • People who have a family history of allergies possess a higher risk.
  • Mexican Americans are at higher risk of developing gallstones.
  • American Indians have genes which increase the quantity of cholesterol in the own bile, also possess the maximum rate of gallstones in America.
Individuals With Specific Health Conditions

Treatment Options for Gallstones

The Emotional Journey of Weight Loss

Relationship challenges and divorce after weight surgery isn’t uncommon for patients to experience. Considering the monumental lifestyle change, adjusting to life after surgery is an emotional journey. Aside from rapid weight loss, many patients have hormones that are off-balance that are difficult to control.

The emotional rollercoaster that some patients face can lead to relationship changes, problems, or divorce after weight loss surgery.

Relationship or marriage tension is common with any lifestyle transformation. The spouse often finds themselves standing on the sideline watching the success without changing themselves. Regardless of how happy they are for you, they may feel threatened by your success.1

In order to effectively bounce back from obesity, you must be ready to face a challenge. You didn’t put on excess weight overnight, so the process of weight loss surgery takes time, but it is well worth it.

When Is It Too Late For Diet and Exercise?

First off, it is never too late to give a solid try to any diet or exercise program. Diet and exercise are effective, although when you reach a certain amount of excess weight, it can feel nearly impossible to do it without help. Exercise and Physical Activity Statistics

The percent of American adults who exercise at least 30 minutes per day amounts to less than 5%.4 It is not a matter if you exercise, it is a matter of when. 57.25 million Americans had a gym membership in 2016 paying an average monthly cost of $58/month. Yet a massive 67% of membership owners never used their gym membership, without even including the members that use their membership only once or twice ever.5
