Getting Back to Nature – Marshall Hepner’s Weight Loss Surgery Testimonial

 “My life before surgery was difficult at best” Marshall explains. At 51 years old, Marshall was 406 pounds and found simple tasks, like tying his shoes, very difficult. His doctors pointed out serious health risks that came with him being overweight and he knew he needed a lifestyle change. After trying out a few, he decided to start a group of his own, “Backcountry Bariatric”. This group is open to people who have had weight loss surgery or are looking to lose weight. They share a common interest in nature and getting exercise outside of a gym in the great outdoors. Members in this group keep each other motivated by acknowledging milestones and giving each other positive encouragement. After just over a year, the group has grown to nearly 400 members from around the world!

Marshall describes the overall process “not easy, but simple”. If you follow the rules and commit to a lifestyle change, it can change your life. Marshall no longer has to take any sort of medication for diabetes. He has more energy and is able to enjoy activities that he wasn’t able to do before, such as hunting, fishing, and long walks with his wife and dogs. The gastric sleeve has had a positive influence on his life and he often recommends both Dr. Cernero and the surgery.

Everything You Need to Know About Low-Carb Diets

Our 2018 Weight Loss & Dieting blog series continues! This month we’re tackling low carbohydrate diets, including a typical low-carb diet, the ketogenic diet, and the paleo diet.

Typical Low-Carb Diets

When you eat less carbs, but aren’t following a more specific set of rules that comes with a paleo or ketogenic diet, you are following a low-carb diet. They can also be called low-carbohydrate or carb-restricted diets.

Low-carb diets are usually centered around meats, fish, eggs, nuts, seeds, vegetables, fruits, and healthy fats. The goal is to limit the carbohydrate intake while maintaining a relatively high protein intake.

This means eating fewer grains, potatoes, sugary drinks, and junk foods.

Can a Typical Low-Carb Diet Help You Lose Weight?

Normally, your body burns carbohydrates for its main source of fuel. During digestion, carbohydrates are broken down into simple sugars, which are then absorbed into your blood stream. Once in your blood stream, these sugars are called glucose.

When the level of sugar in your blood rises, your body triggers the release of insulin, which helps the glucose enter your body’s cells. Once in your cells, some of the glucose is used to power your body, fueling your activities. Leftover glucose is stored in your liver or muscles for later use, or converted to fat.

So, when following a low-carb diet, the idea is that the limited carb intake will naturally reduce the amount of insulin produced, which will then cause your body to burn stored fat for energy instead of carbohydrates. The more fat that is burned, the more weight you lose.

Are Low-Carb Diets Safe for Diabetics?

It turns out there are many studies supporting low-carb diets as a treatment for diabetes. Of course, before making any major changes to your diet, speak with your doctor.

The Ketogenic Diet

The ketogenic diet, often referred to as keto, is one that places an even greater restriction on carbohydrate intake, with emphasis on healthy fats.

When following a ketogenic diet, the goal is to limit carbs so much that the body goes into a metabolic state called ketosis.

Ketosis occurs when insulin levels drop very low, and the body releases large amounts of fatty acids from its stores of body fat. These fatty acids are then transferred to the liver, where they are consequently turned into ketone bodies.

Ketone bodies, also called ketones, are then used to fuel the body instead of carbohydrates.

Is the Ketogenic Diet Safe?

Many people promote keto as a safe and effective method of weight loss, however, there are many people who argue against keto as well. Again, please confer with your doctor before starting the ketogenic diet on your own.

What is the Paleo Diet?

The name of this diet is derived from the Paleolithic era, the period of time when modern humans, or Homo sapiens, evolved. The idea behind this diet is to consume foods that were most likely available to humans alive at the time, before the agricultural and industrial revolutions.

This diet includes meats, fish, eggs, vegetables, fruits, nuts, seeds, and tubers, while excluding processed foods, added sugar, grain, legumes, and dairy. Imagine eating how hunter-gatherer-scavengers would have around 50,000 years ago.

Is the Paleo Diet Healthy?

There are studies linking the paleo diet to weight loss, reduced blood sugar levels, and improving risk factors related to heart disease. As we said before, please consult your doctor before making any big, sudden changes to your diet.

Because weight loss surgery (at Cernero Surgery, we perform sleeve gastrectomy and Lap-Band) involves a major, permanent change to your body, there are certain criteria that must be met in order to obtain eligibility for one of these operations.

While these surgeries offer many benefits – such as the reduction of weight-related health problems: type 2 diabetes, high blood pressure, heart disease, depression, high cholesterol, and sleep apnea – there are risks involved as well. This is why health care teams must ensure a patient meets all eligibility requirements before a surgery is conducted.
 
Are You Eligible for Weight Loss Surgery?

In general, weight loss surgery (gastric sleeve or Lap-Band) is an option for you if you meet the following criteria:

  • Your body mass index (BMI) is 40+,
  • Or your BMI is 35+ and you experience severe weight-related health issues (type 2 diabetes, high blood pressure, etc.),
  • And past efforts to lose weight with diet and exercise have failed.

When the above guidelines are met, there is still more that needs to be done to determine whether you are a candidate for weight loss surgery.

A team of health care professionals will evaluate the following in addition to the physical requirements listed above.
 

Your Current Psychological Condition
If you suffer from certain mental health disorders, such as substance abuse, eating disorders, or bipolar disorder, your doctors, and possibly a psychologist, may postpone surgery to ensure these stressors are properly cared for and will not interfere with your weight loss surgery and recovery.
 

How Old You Are
There is no real age limit on potential weight loss patients, but in the past, these procedures were considered too dangerous for teens and older adults. Recent studies have revealed that bariatric surgery can be an effective and safe method of weight loss for people in a wider age range suffering from weight-related health complications. That being said, our surgeon will still take your age into account when considering you as a candidate for surgery.
 

Are You Motivated?
Weight loss surgery has proven to be a great method for weight loss, but it is by no means an easy process after the operation is completed. Your health care team will evaluate your willingness and capability to adhere to the recommended changes in diet and exercise. If you don’t follow the rules after surgery, you may cease to lose weight or begin to gain it back.

How to Stick to Your New Year’s Resolutions

Now that January is almost over, it is a good time to check in on our New Year’s Resolutions. Are you still holding on? Most people aren’t. Did you know that it’s estimated only 8% of people actually maintain their resolutions for an entire year? Why is it so difficult to develop new habits? What happens to us?

January 1 signifies the start of a new beginning, we set goals, three weeks go by, and we’re back to our regular old habits. For most people, this occurs because we were too ambitious, became discouraged, then simply give up. In 2018, let’s put an end to the cycle of resolution-ditching.

Instead of making multiple massive changes, start small. For example, if you wanted to start exercising more, make time in your schedule to go to the gym, take a walk, or go on a jog two times a week. After a month, increase that to three times a week. It’s much easier to stick to a resolution when you give yourself attainable goals.

It’s also important not to beat yourself up if you make a mistake. Changing human behavior is a lengthy process, so you can’t expect yourself to achieve perfection immediately.

This year Cernero Surgery is setting out to explain, weigh the pros and cons of, and analyze a particular set of common New Year’s resolutions: weight loss and dieting.

There are many different diets out there – ketogenic, vegan, low-carb, juice cleanses, fasting – that everyone seems to tout as the number one regimen. Who can you trust? Is there truly one diet that is superior to the others?

In 2018, we’re going to set the record straight.

To begin the series, let’s discuss plant-based diets.

Everything You Need to Know About Plant-Based Diets

There are a variety of diets that center around plants: veganism, vegetarianism, pescatarianism, pollotarian, and Mediterranean. Plant-based diets are known for improving cholesterol, reducing blood sugar levels and cardiovascular risk, and helping with weight loss.

What Does It Mean to Be Vegan? What Do Vegans Eat? 

Veganism is the most intense of the plant-based diets. Vegans exclude all meat, eggs, dairy products, and all animal-derived products and ingredients. Many vegans extend this lifestyle beyond their eating habits and will also avoid the use of clothing, household products, and more, that were created by testing on animals, or with animal products. For example, they would not wear a leather jacket.

From an outsider perspective, you might be wondering, what do vegans eat? It seems like there’s not much left, but that’s really not true. Vegans eat grains, beans, vegetables, and fruits, and the many combinations of those foods.

There is also an increasing number of substitute products out there, like veggie burgers, vegan cheese, and non-dairy yogurt. Many vegans enjoy tofu, but it’s definitely not a requirement to eat it if you decide to go vegan.

What Does It Mean to Be Vegetarian? 

Vegetarianism is a less strict form of veganism. While vegans do not any eat meat, eggs, or dairy products, vegetarians are a bit more flexible. They try to avoid any foods that come about from the slaughter of animals. For example, some vegetarians may continue to drink cow’s milk, but abstain from eating a burger. There are also a few sub-classifications:

Lacto-ovo vegetarians: They avoid all animal meat, but continue to eat dairy and eggs.

Lacto vegetarians: They avoid animal meat and eggs, but continue to consume dairy products.

Ovo vegetarians: These vegetarians avoid all animal products except eggs.

What is Pescatarianism? What is a Pollotarian?

If someone follows a pescatarian diet, that means they follow a vegetarian diet, but they also eat fish and seafood. Many people choose this diet so they can gain the benefits of a plant-based diet in addition to heart-healthy fish.

A pollotarian is someone who eats poultry, fish, dairy products, and eggs, but abstains from eating red meat from land mammals like cows, pigs, and lamb. This diet is often used as a transition period from a typical omnivorous diet.

What is a Mediterranean Diet?

A Mediterranean diet centers around how the Greeks, Italians, and Spaniards eat. The goal is to eat plenty of vegetables, fruits, nuts, potatoes, whole grains, seafood. Then in moderation, poultry, eggs, and cheese. Then only rarely, red meat. And finally, foods to avoid completely: sugar-sweetened beverages, process meats, and refined foods.
Studies have shown that a Mediterranean diet leads to weight loss, helps prevent heart attacks, strokes, type 2 diabetes, and premature death.

Can a Vegetarian Diet Help Me Lose Weight?

There are many reasons to start a vegetarian diet. Some cite the fact that the production of vegetarian food is more environmentally friendly and more ecological sustainable. Others stop eating meat for ethical or religious reasons.

The most popular reason is the health benefits offered by a plant-based diet. In multiple studies, researchers have found that people who identify as vegan, vegetarian, or pescatarian have lower BMIs (Body Mass Index) than those who eat meat. They also have healthier levels of cholesterol, a lower risk of developing cancer, and a lower risk of cardiovascular issues.

When it comes to weight loss, that is entirely up to the person. Simply switching to a vegetarian diet does not automatically mean you are healthier. Did you know Oreos are vegan? If you only ate Oreos, you would technically be vegan, but you would obviously be extremely unhealthy and experience a variety of undesirable side effects.

However, if you focus on eating whole foods like beans, legumes, fresh vegetables, and fruits while monitoring your caloric intake, you have a much better chance of losing weight.

Complete pre-admissions testing, including psychological evaluations, nutritional evaluations, and medical diagnostic tests.

2. Work with your physician to arrange and begin following a supervised diet plan.

Begin eating for health and not just flavor or pleasure. Changing your diet before surgery makes it easier to lose weight post-operation, as you’ll already be used to eating healthier and controlling portion sizes.

3. Stop drinking alcohol, stop smoking cigarettes, and begin exercising a little bit more.

After surgery, alcohol will have a much different effect on your body – it’ll be easier to become intoxicated, your blood sugar may go haywire, and it can put you at risk of several severe health issues. Smoking increases the risk of blood clots during surgery, and studies suggest that tobacco smokers fail to complete pre-op programs at twice the rate of non-smokers.

4. Two weeks out from surgery, you should already have been eating and drinking as if you’ve had surgery, you should be walking or exercising for at least 20 minutes per day, and you should have authorization from your insurance for payment.

Before the big day, make sure you have the following ready:

  • Planned time off from work to recover, typically around 4-6 weeks
  • Post-op prescriptions from your doctor
  • Care for your child or children during surgery and recovery
  • Transportation to and from the hospital
  • Someone to be with you 24/7 during the first week of recovery
  • Food, drink, and vitamin preparation
  • Comfortable sleeping arrangement at home, your body will be very sore
