Abigail and Brittany Hensel are about to turn sixteen. They’ve shared every single moment of their lives, but it wasn’t a matter of choice. These twin sisters are joined for life. Abby: “We wanted to make this documentary so people wouldn’t always have to stare and take pictures because we don’t like it when they take pictures”. They have a condition known as dicephalus.

Abby and Brittany were born with just one body between them. The only known surviving dicephalus conjoined twins. They’ve spent their lives in a small close-knit community, completely protected from prying eyes. The girls live on a farm in Minnesota with their younger brother and sister, Coty and Morgan, and their mother and father, Patty and Mike Hensel. As they reach sixteen, on the brink of womanhood, they decide to show the world what it’s really like to be joined for life.

Every moment of their lives requires the twins to cooperate with each other, and they appear to be totally in sync, but that doesn’t mean they always agree. Brittany describes Abby as bossy and outspoken. Abby describes Brittany as having been shy but becoming more confident, she likes to take her time with things.

Conjoined Dicephalus Twins

Abby and Brittany were born on March 7th, 1990 and the fact that they were conjoined twins was a complete shock to everyone. Pre-natal scans had shown only one baby.

Abby and Brittany had only a one in thirty million chance of surviving the first twenty-four hours. Their survival would all depend on how their organs were configured within their tiny body.

Queen Charlotte Hospital in London specialises in the most complicated twin pregnancies. Professor Nick Fisk, of the hospital, has followed the case of Abby and Brittany Hensel since they were born. “I have never seen, previously, a set of surviving dicephalus twins, so they are extraordinarily rare and the only ones, to my knowledge, alive today.

They have two separate hearts, one in each side of the chest, and each has its own set of lungs. The medial lung seems to be fused in the middle, as one might expect. This is consistent with an increasing degree of fusion as one moves down the body. Abby and Brittany had three arms at birth, but the third was a useless mid-line arm that didn’t function so it was removed when they were very young”.

To the medical world, Abby and Brittany were an astonishing case, confounding all expectations of dicephalus conjoined twins. With their bodies so closely linked it was clear that separation was out of the question.

Sharing a Body

Each twin controls their own half of the body and each twin has no sensation of touch from the arm of the other. Nobody really understands how this works in practice.

At the age of twelve, it was discovered that they had a life-threatening condition. Abby and Brittany’s rib cages were fused together and their spines were growing outwards rather than upwards. The girls had to undergo a major operation to stop their lungs running out of air. After being discharged from hospital they had gained two inches in height and were fit to live life to the full.

Professor Fisk explains more “Their degree of motor coordination is really quite extraordinary, it’s not as good as you or I but it’s pretty amazing. How does that happen? There’s absolutely no degree of fusion of their spinal columns; they are completely separate. This can be seen in the X-ray of their pelvis. They have a single pelvis but two sacrums, which is the lower part of the vertebral column, so I don’t think anyone can explain how that happens”.

After their sixteenth birthday, they plan to do what every American teenager does, learn to drive. However, this will require a great deal of cooperation and coordination. Abby controls the legs so will operate the pedals, they each control their own arm so both will steer but no-one really understands how the visual information is processed.

Dual Driving

Who should get the driving licence? To solve this problem they each take the test and having passed, each receives their own driving licence.

The family have always refused to have any non-essential tests carried out on the girls, despite great interest from the medical profession. While the girls are unique, the family wants to treat them just like everyone else.

Professor Fisk “There would be a very long queue of scientists, with very modern techniques of imaging brain and nervous system function, who would love to find out how the girls coexist”.

A young girl in India badly burned as a toddler, her fingers had fused together and curled into a knotted ball. Her shepherd family could not afford surgery, but they had heard of a remarkable young boy being called the child surgeon. Akrit Jaswal was only seven years old when he operated, successfully, on the eight-year-old girl to release her fingers.

Akrit Jaswal had a reputation, in the region, for being a medical genius. This young man has been shown to possess an I.Q. of an impressive 146. This is the highest I.Q. of any boy his age in India, a country of over one billion people.

He has focused this phenomenal intelligence on medicine. And now, at the age of twelve, claims to be on the verge of discovering a cure for cancer.

An early developer, Akrit was walking and talking by the time he was 10 months old. He was reading and writing by two. And, reading Shakespeare, in English, by the time he was five. Akrit is now talking about his theories for oral gene therapy in the fight against cancer.

He has been sponsored and mentored by Mr B. R. Rahi the Chairman of Secondary Education in Dharamshala. The child surgeon is studying for a science degree at Chandigarh College. At twelve years of age, he is the youngest student ever accepted by an Indian University.

Akrit’s father left the family a year ago, depressed and exhausted. Trying to get the bureaucratic Indian Government to acknowledge his son’s intellect, took six long years. Without this acknowledgement, he had little chance of securing funding for Akrit’s cancer research.

Is it possible that this young boy knows something the medical profession does not? Throughout history, scientific breakthroughs have come not only from the established, the learned, and the scholarly. But, also from single flashes of insight and inspiration.

Akrit Jaswal is not phased by his fame and is used to meeting government ministers and press representatives. For ordinary people meeting Akrit, it is very different. When he is in town, they gather for an audience. They come with prescriptions and medicines, seeking advice. Some come with ailments and injuries for a diagnosis. They come to see a doctor, a healer. They come to see a guru, and because this is India, there is always a spiritual dimension.

Expert Support

Akrit Jaswal may be famous but, will he be the one to unlock the secrets to a cure for cancer. Staff at Imperial College London invited him in for some tests to establish his credentials. Where he was talking super-mechanisms, genes and therapies with scientists at the cutting-edge of cancer research.

Drs Patel and Djamgoz

The aspiring child surgeon must convince Professor Mustafa Djamgoz, a world-renowned research biologist. Akrit must also persuade his colleague Mr Anup Patel, a consultant urological surgeon. That his ideas are realistic and worth pursuing.

The inquisitors become his friends. Mr Patel and Professor Djamgoz are keen to foster Akrit’s enthusiasm. They are keen to protect him from disappointment and willing to guide him on his way.

Professor Djamgoz says of Akrit: ” He is generating ideas based upon what he knows, in an idealistic sort of way, without being in full grip of reality, without knowing how difficult it is to turn the ideas into practical realities”.

Just how intelligent is Akrit? Team Focus, the UK’s leading I.Q. analysts agree to test him. For Akrit this was to prove a disappointment. The young man had exceptional results in verbal and numeracy tests. But, poor practical tests let him down. Particularly in the area of pattern matching. Because of this wide range of results Team Focus chose not to give him a final rating.

The Psychology of a Genius

Rosemary Facer, a childhood psychologist, put forward the theory that Akrit had been an early developer, accounting for the good results. However, because of this early genius, he had missed out on later schooling, accounting for the poor practical results. These results do not affect what Akrit may achieve, but he needs help, a wise friend to talk to.

The Professor’s analysis is that Akrit needs to obsess less and enjoy more. He thinks Akrit shows great potential. But, potential needs proper guidance.

Akrit returns home to India, slightly maturer, a little more realistic. Akrit Jaswal, the child surgeon, is a precocious young man. Furthermore, the confidence of this young man means that he is still sure he will find a cure for cancer.
