Does anyone have an online system for bedside safety checks of emergency equipment (eg oxygen, suction etc)? We currently have a paper based version where staff have to initial on their shift that they have been checked. Some things are checked on a shift by shift basis, some daily and some weekly dependant on what it is and the area of the ED or services. Hey all, I’m aware that putting this post out at a time we’re there’s controversy over mid-level encroachment will gain some negative responses. But I want the brains trust perspectives on whether or not paramedicine should or may become apart of medicine. 73,750 Came in with acute worsening of progressive SOB over the last two weeks. Sats in the 70s 15L NRB. SBP 160s. Started bipap and nitro drip and in about 15 minutes he was looking much better. Subsequently found a pna and weaned him off the nitro. Several hours later he actually needed pressors as I REALLY did not want to give this dude much fluid. Hey guys, I am almost starting my second yr in residency. I feel like I get a lot of push back from consult services. This has led me to a point where when an attending tells me he/she wants a consult, I get anxious because I know I might get push back (even though I understand to get one). I will say sometimes consults really do not make sense and other times I feel like neither the attndg nor I know what to do with a patient so we anchor on a consult to get things done. ER attending here, about 5 years in. I’m starting to notice my long term memory has been awful, i.e. trouble remembering certain days/events/conversations and even long periods of time. Even the present moments feel a bit fuzzy. I’m curious if any other ER vets have noticed this. I figured it could be: 1. The switching from nights to days is affecting me 2. Too much phone time 3. How would you guys handle this patient? 60F presents with palpitations. New onset afib RVR in the 160s. Pressures around 90. Hx CHF with an EF of 25% on last echo which is about a year ago, AND a history of pHTN. IVC looks roughly normal but with BLE edema. No B-lines but is complaining of some SOB. Had a TIA 1 week ago, not on any thinners. Obviously work up for the underlying cause, but then managing the RVR is where it gets a little tricky because everything seems contraindicated for one reason or another, not quite unstable enough to shock. I worked in an ER from the hours of 4-1am and 5-2am absolutely loved it since I was a night owl. Stupidly, I decided to do 7pm-7am shifts 3x a week and flip my schedule on my off days… now I have insomnia. I either fall asleep at night and get super fragmented sleep or fall asleep later in the night at like 5am-6am. Occasionally I get unlucky and my body wants to throw a fit and I just can’t sleep at all. I’ve been off nights for 6 weeks at this point and worked then for 7 months so idk why this is still happening.
