How do you detect caries clinically?

So I am in dental school in the U.K. I am a qualified dental hygienist and dental therapist - as a therapist we can do fillings so I have been taught caries and fillings before at a different school. In my previous school we were taught to probe the lesion, if sticky its active decay. In my current school we are told not to probe as we may cause cavitation, and so are expected to assess the lesions just by drying the area and looking, as well as using radiographs obviously. Do y’all use templates? Anyone have key words/phrases they always have in their crown appointments, or bridge appointments, or etc? SOAP notes is how I write my notes. But wondering how I can improve to have really air tight documentation. “Risks, benefits, and alternatives associated with recommended treatment plan discussed in detail with patient. Something I have been debating as a new grad is how long a solid filling can last in a patient who takes good care of their teeth. We all know natural tooth is stronger than any restorative. I feel like in my experience (limited experience haha) a well-placed occlusal restoration can last seemingly forever. When it comes to a class 2, assuming good caries removal and good restoration placement, and the patient keeps it clean with an ok diet, do people see those lasting a while?
