Case 1) This 82 year old man with no past history of skin cancer attended to show me this lesion which he said had appeared in the last 2 months.
What is your analysis, your preferred diagnosis and the percentage likelihood that this is going to be right?
I know some of you hate putting down percentages but I think it is a really interesting (and useful) exercise.
Case 2) Courtesy of my colleague Sinead Dean. A 59 yo woman who became aware of this 3 months prior when her towel caught it.
Again I would be interested in your preferred diagnosis and percentage of confidence ..read more

I never did well in Pascale’s quizzes but like many of you, I always liked them and I certainly miss her presence in the blog.
This is my humble tribute to Pascale.
I am travelling to the Gold Coast for the Congress so feel free to comment and I will answer when I can.

Today’s cases are from my colleagues Dr. Karen Viduya and Dr. Sam Axford
Case 1. A fascinating case. 79 year old lady who states this lesion has been present since birth. We had an initial histological diagnosis, then a revision of the diagnosis after our request for an in-house review.
Case 2. A 56 year old man who has a lesion on his left cheek. The diagnosis might look simple, but there may be a bit more to this lesion than first impressions? Yep it is a bit tricky.
Case 3 A 44 year old man with many naevi, some atypical as can be seen on his TBP image  ..read more

I found this lesion no the forearm of a 79 year old man. It was slightly raised and a bit firm. I thought it was most likely a bruise, but something smelt fishy. I did a punch biopsy. What did I find?

Case 1 Lesion in the scalp not present at birth in a 2 years old girl. What do you think it is and what would you do about it?
This man is 55 and has had 2 invasive melanomas in the past. He has numerous atypical nevi. This was one on his abdominal wall that I photographed and then when filing the photos I thought I had better excise it! Was I justified by the subsequent histology report?
Case 3 A lesion on the temple in a 75 years old male. No recent changes. Nice vessels. What is it?

One of those cautionary tales. I thought a good illustration of the capabilities of lentigo maligna and surgical margins. This is though in the setting of some immunosuppression.
Bit of a saga sorry but this 67 yr old man, who is now a retired builder, first presented back in 2015 with a pigmented lesion on the left temple. Complicating features were his history of psoriatic arthritis on methotrexate and humira.
Diagnostic excision undertaken and reported as lentigo maligna with a positive margin at 12 o’clock.
Wider excision undertaken by myself, I don’t have a photo of that but in my op no ..read more

This 80yr old gent with a history of NMSC presented with this growing painful nodule on the superior helical rim over several months. It was extremely tender to touch making examination of the lesion difficult. Clinical features and dermoscopy which showed keratin structures ,ulceration ,white circles and dot vessels favoured a diagnosis of SCC. The distance of excision with 4mm clinical margins was around 25 mm.
How would you manage from here ?
