I'm starting the process of working as a 1099 independent contractor, strictly as a 1st assist in Orthopedic surgery. If anyone has experience with this topic, how do you have reimbursement set up for your services? do I set up an agreement through a Medical Billing company to bill insurance for my fees assisting in surgery, do you set up a contract to bill the hospital at say an hourly rate/flat fee, or do you bill the surgeon an hourly rate/flat fee?  Also, is there any difference when billing for your services through a hospital vs a surgery center ..read more

Hello. I am a PA that is going to be working at a surgical office as a new job. From what the SP has told me, they consult and walk into each case with you and review the case with you for shared plan, and because of this they bill under the SP’s NPI. He said if he gets called into surgery and he’s not there and we see a patient then we can bill under our npi.    I wanted to clarify as I have previously only worked in EM and billing is very different there, that if he is consulting on every case and reviewing the note and putting in an attestation, then I assume there is no legal/bil ..read more

11300 codes

Hi I work in dermatology , recently have been billing for 11300 codes to remove warts and isks. I’ve done them for two weeks or so and now the billing department  is requesting pathology? I’ve been billing them out as warts or isk codes so why is pathology even needed? Was I doing it incorrectly  ?  I can see why you would need path for 11100 codes for d48.5 dx but for 11300 codes I don’t see why this would need a pathology if it’s for the wart or isk codes   any advice? Last year the APP staff became aware of some billing irregularities related to incident to billing and how things were showing up on our fee tickets.  After pulling up Medicare guidelines we found that our SP was not seeing Medicare patients for their initial visit.  This was brought to our office manager in an open meeting to which she stated "there is a loop hole for this."  I should note that our office manager / COO styles herself as a billing expert, and we were told that she has vast experience in billing and was certified as a coder and biller.  After two months it w ..read more

I currently work in orthopedic practice that is hospital employed. I've got 5+ years of ortho experience. My current salary is fair but my bonus compensation is insulting. I made a little over 2k in bonus last year. The hospital claims my reimbursement for my chargeable services was only $177k last year yet they can't provide me a breakdown of that number. I make $1wRVU that the physician can't bill for. PAs in ortho with my level of experience easily get 10% of their total compensation from bonus. Hello everyone! I started working in dermatology last year and unfortunately have run into a lot of issues with the company itself as far as billing and collections. It has been almost a year since I have been hired and still am not credentialed for Medicaid as a whole.
