MSHA released its preliminary report on August 5, 2022 regarding a fatal mine accident that occurred in August of 2021.  Timothy Collins, a 53-year-old contract truck driver with over 10 years of experience, was fatally injured while conducting a pre-operational examination of a vehicle.  According to the investigation,  Mr. Collins was killed when a dump truck he was inspecting suddenly rolled over him.  The parking brake failed and the truck was parked on a grade. CLARKSBURG, WV – A tragic workplace fatality occurred in March 2022 in the Clarksburg area at around 2:30 pm.  Several pieces of heavy excavation equipment were being operated in the area of East Main Street in Clarksburg, West Virginia.  Wayne Burchard, age 75,  fell in front of a moving excavator and was run over by the machine.  Mr. Burchard was a truck driver who was a contractor from Empire Builders in Parkersburg.  He had been hired by the City of Clarksburg to work on a demolition site. KANAWHA COUNTY, WV: A man has been charged with Driving Under the Influence Causing Death after deputies say he caused a crash under the influence that led to a woman losing her unborn baby. A Harrison County man who was on a work release furlough weekend is facing charges of DUI causing death with reckless disregard after his female passenger was killed in a Sunday morning crash in Sardis. He currently in the North Central Regional Jail on $50,000 bond.
