Exercise, while one of The Most Important self management approaches for persistent pain, is not an easy sell to someone who is experiencing pain. Especially not if that exercise looks like huffing and puffing, hauling on bits of metal in a gym, or wearing lycra. Not to mention the “sports drinks”… Those things aside, exercising is a good thing. You heard it from me, and I have declared my body an exercise free zone! The thing is, what kind of exercise, for what purpose, and how to get introduced to it. Personally I’m a fan of exercise that achieves something else other than “getting fit”. I ..read more

Fat Pad Anatomy Generalized pain in the front of the knee, also known as anterior knee pain, can have many causes. One cause is impingement of the fat pad. The fat pad is a mass of fatty tissue that lies below the kneecap (patella) and behind the patellar tendon. With the run up to the London Marathon I am seeing more and more people complaining of the dreaded runners knee. This is all about the ITB/runners knee syndrome The iliotibial band (ITB) runs down the outside of the leg from the side of the pelvis (ilium) to below the knee where it attaches to the upper outside part of the lower leg (tibia and fibula). It is a tough fibrous band of connective tissue which blends with the small tensor fascia lata (TFL) muscle. Iliotibial band syndrome, also known as runner’s knees, occurs when the ITB becomes overly tight and rubs over the femoral condyle (low ..read more

Despite being a less common cause of low back pain, sciatica is still something I regularly see as a general internist. Primary care doctors can and should manage sciatica, because for most individuals the body can fix the problem. My job is to help manage the pain while the body does its job. Sciatica refers to pain caused by the sciatic nerve that carries messages from the brain down the spinal cord to the legs. The pain of sciatica typically radiates down one side fro ..read more
