Rates of surgical success and failure are a key component of informed consent. Residual fragments, even <2 mm, after surgical management of kidney stones carry a substantial risk of progression,1 with more than 20% of patients requiring additional intervention for fragments <4 mm.2 The burden of residual stones can be further extrapolated from Sorensen et al’s recent randomized controlled trial that showed a 4-fold increase in stone-related events in patients who did not receive concurrent treatment of small nonobstructing stones during management of a primary ureteral or renal stone ..read more

Retrograde intrarenal surgery has gained significant acceptance for the treatment of upper urinary system stone disease. Many studies reported high rates of efficacy with low complication rates.1 These findings encouraged the clinicians to perform retrograde intrarenal surgery (RIRS) and even guidelines recommended this technique with high grade of recommendation. On the other hand, the stone-free rates varied widely in literature.2 The main reason of this situation is the lack of standard definition for stone-free status after RIRS ..read more

Extravaginal torsion (EVT) is a rare type of testicular torsion that usually occurs in neonates. The primary type of testicular torsion that occurs in adolescents is intravaginal torsion (IVT). In this case report, we describe the first case of EVT reported in a 16-year-old male with a contralateral bell clapper deformity and subsequent surgical management using a tunica vaginalis flap (TVF) and bilateral orchiopexy. In discussion of this case, we examine possible anatomical causes of EVT and suggestions for appropriate surgical management ..read more

We engage in research for many reasons, but this manuscript held significant personal interest for me. Amid the rigors of residency training, I experienced the loss of a very desired pregnancy. When the Dobbs’ decision leaked in May 2022, I was an attending. One of my first thoughts was how differently my miscarriage experience could have been as a resident. In 2019, I had the financial resources to travel out-of-state if needed, but I was allowed to recover in my own home. I was able to receive the necessary medical care with speed and dignity ..read more

Re: Predictors of Excellent Urology Residents at the Time of the Urology Match
Urology - Gold Journal

We read the article by Thompson and colleagues1 with great interest given the competitive nature of urology residency and desire of all programs to match high quality candidates who will achieve success as residents and, later, independent physicians. The timing is quite apropos with resident interviews currently in full swing. The manuscript utilized a logistic regression model trained to predict resident excellence, as based on scoring from the current and prior program directors. Based on their multivariable model, no negative interview comments and honors in all core clinical clerkships we ..read more

Editorial comment on: “Clinical Efficacy Evaluation of a Novel Negative Pressure Ureteroscopic Lithotripsy for Ureteral Stones”

There have been numerous advances in the treatment of nephrolithiasis, including miniaturized instrumentation and new laser technology; however, the purpose of the procedure remains the same—to render the patient stone free. Recent publications re-emphasizes the need to render patients stone free with residual fragment size contributing to stone-related events for both percutaneous nephrolithotomy1 and ureteroscopy.2 Both the vacuum-assisted ureteral and percutaneous access sheaths are now commonly used with advertised benefits of a reduction in stone retropulsion, shorter operating room times ..read more

The management of kidney stones, particularly those in the renal pelvis, is a critical aspect of urology. The European Association of Urology guidelines recommend Extracorporeal Shock Wave Lithotripsy or Endourology methods, encompassing Percutaneous Nephrolithotomy and Ureterorenoscopy (URS), for stones ranging from 10-20 mm. Robotic-assisted urological procedures have gained prominence in recent years, promising enhanced precision, and safety ..read more
