I am an 83 year old male with CKD 4 (eGFR 16). Had bladder cancer in 2016, bladder removed, clean/clear, no chemo. Have only one kidney (donated one in 1990). Now have secondary hyperparathyroidism (PTH 137); being treated with calcitriol. B/P normal. Glucose normal, weight – very good (145lbs/5’10.5″) bun/creatinine abnormal. Struggling with decision – transplant or home dialysis. What factors to consider? What would be your advice? Thanks.

It is difficult to give advice without performing a complete history and physical examination. At the age of 83, transplant becomes more difficult and in most cases, a living related or living unrelated donor would be best. If there are ..read more

I have been preparing to donate a non-directed kidney donation. I am 76 years old. Today I received my Iohexol result which is 70. The National Kidney Foundation web site says that 60-89 may mean early kidney disease. And that 75 is average for someone 70 and older. Houston Methodist hospital wants to continue with more testing but it sounds to me that I don’t have sufficient filtration to donate. I would like your insight and advice. Thank you.

Kidney function declines with aging, such that after age 40, kidney function declines at an average rate of 1% per year. Hence, at your age, you would be expected to have an estimated glomerular filtration rate (eGFR) of around 60 ..read more

Is it anything to worry about if a kidney donor has what seems to be a large increase in blood creatinine levels and drop in glomerular filtration rate two years after donation? My levels were fine, if a little outside the normal range after donation. I haven’t had blood work in a year but my creatinine has increased 25 pts and my glomerular filtration rate, which was previously on the low side of normal, is now at 52, dropping 16 pts. It seems a drastic change. I’m seeing my doctor soon..

If my husband’s GFR is going up 14 to 17 with slightly elevated potassium due to Torsemide refill pending, what would be the urgency for the placing of a fistula? Could one work with a placement of fistula in a job that consists of heaving lifting?

I commonly recommend that a fistula be placed when the estimated glomerular filtration rate (eGFR) is less than 20 milliliters per minute per 1.73 meters squared. If there is a living donor available for kidney transplant, then the fistula may not ..read more
