Check out the article, "Why Market Power Matters for Patients, Insurers, and Hospitals" by Atul Grover, MD, PhD from the Association of American Medical Colleges (AAMC): https://www.aamcresearchinstitute.org/our-work/data-snapshot/why-market-power-matters Excerpts: Health systems argue that consolidation has been driven by financial pressures as well as opportunities to eliminate excess capacity, integrate care for patients, gain efficiencies, and improve leverage for negotiating with private insurers, among other reasons. Indeed, the extensive consolidation of the insurance industry has ..read more

Hi, I’m currently trying to obtain another EM sub-I for August (I have one for July). I know I need two SLOE’s. My first question is August the latest you can do a rotation to receive a SLOE in time for residency application? My second question is how are subspecialty (US, tox) SLOE’s looked at and would it be okay to have one general EM SLOE and one subspecialty SLOE? I was offered a subspecialty rotation for August since gen EM was full, but am also still waiting to hear back from a few other schools about my app for EM sub-I for August and am nervous that while waiting for responses for th ..read more

Compartment syndrome question

Emergency and surgical nurse here. I have a patient who had a fasciotomy from mid thigh to mid calf, (neck fasc, had a debridement done). He has a vac dressing running continuous on this wound. He started to develop severe pain to his ankle and foot, it was taught, shiny, cap refill was slower than earlier in the day and pulses were barely palpable with a Doppler. I have never seen compartment syndrome IRL, but this seemed like a possibility and I didn’t want to take any chances. I called the resident who ended up calling the surgeon. The surgeon basically called me an idiot and said it was i ..read more

Ethically speaking, do you have experience with telemed platforms that use coercion and manipulation in effort to alter your clinical judgement or maximize profits? What’s the worst you’ve experienced?

This has happened to me so many times. It seems telehealth platforms make it hard for providers to communicate with one another. I started this sub to provide a space for virtual care providers to support one another. https://www.reddit.com/r/telemedcontractors/s/NjbFahwdM5 submitted by /u/3-2-go [visit reddit] [comments ..read more

Days off my hands feel great. Days on I lube the shit out of my hands, use my topical kenalog, and will wake up from sleep with dishidrosis between my fingers What are my dudes and dudets doing to protect their sensitive hands whilst getting boned by our corporate overlords? Anybody else deal with this on shift, or has anyone made the jump to clobetasol, TCI’s, or dupixent? (Uptodate rabbit hole). submitted by /u/Noms4lyfe [visit reddit] [comments ..read more

(Non-US) FM resident in the process of applying to do additional residency training to be certified in EM. I’m doing ER electives and really enjoying them, but lately I’ve been feeling a bit discouraged because of my limitations. Intubations, reductions, casting - a lot of things that can require some force and/or not the most ergonomic movements, I struggle with because of pain. For the last few years I’ve had chronic generalized joint pains, but especially in the wrists and shoulders. Fingers are also challenging, since if I’m not careful they can hyperextend a bit too far and can get locke ..read more
