I am Canadian, with long waits where I live for biopsies. Consequently I am looking for information on procedures available in several countries. I am looking at transperineal fusion biopsies. These are superior to transrectal because of a lower incidence of infection. Is anyone familiar with hospitals in Mexico that offer this procedure. Thanks.

I have just started this journey, and am deciding on treatments now. 60 years young with a Gleason 5+4 PSA hovering around 7. My PSMA PET showed one lymph near pelvis for sure, and possibly a rib neat top on left side. My medical oncologist ordered a bone scan, which showed nothing on the left side, but a possible spot on the right side. He thinks it is nothing, and puts me at Stage 4A. The Radiation Oncologist disagrees and thinks it is in the bone, so has me at Stage 4B. All agree that surgery is not an option.

Galleri cancer test indicated positive for breast and prostate
Follow up PSA test showed 0.4 (last year 0.3)
breast mammogram and ultrasound negative

My father (61) had PSA above 500, MRI showed cancer had spread to bones and was causing him spinal cord compression which had made his legs weak and he became unable to walk within 2 weeks, Oncologist suggested for radiation treatment (single session) to treat spinal cord compression which he just had yesterday and he put my father on abiraterone. Oncologist didn't even ask us to get biopsy considering the MRI reports and CT scan reports. Further, he said TURP or prostatectomy should be put on hold and spine is of more importance as of now. My dad had a prostate biopsy which was positive for Gleason 8 high grade adenocarcinoma. He did the test due to being symptomatic for prostate cancer. Previous tests were showing conflicting Results.
His urine biopsy was positive for high grade prostate cancer.
His digital rectal exam was negative.
His PSA was normal.
His MRI showed low to intermediate risk.
I’m confused what all this means. And I’m scared.

There was a thread on this page and I shared this same post there. I thought I should place it on the front page so more people see it, if anyone sees any mistakes please correct it. Hope it helps in giving an idea about the condition
Screening is done after the age of 50 and is done via PSA & DRE. DRE use is controversial and not that useful but still done for the sake of completlness but is very unreliable. PSA can also be unreliable since at times it is low while there is a cancer but it is significantly better than DRE

Background - During routine psa test ,it was highly elevated 1st test -100 2nd test (after few days )-70
Multiparameter MRI done -Pirad 4 Psa test 3 after a week of antibiotics 3rd test psa -15
Biopsy was next - The biopsy was negative
Conclusion -The psa was highly elevated because of an infection .However the prostate is 69cc hence doctor concluded I must have a simple case of BPH .
He suggested surgery however I am apprehensive and would like to try medication for atleast a year .
Urinary flow is just fine ,taking silodosin twice a week . What do you guys think ?what treatments of BPH are ..read more

How long after you confirmed PC diagnosis did it take to schedule RALP?
A friend said 5 weeks - we live northeastern US with 4 major cancer centers all no more than a days drive away.
Any experiences?

Done yesterday. Read many posts re: biopsies; rectal, perineal, fusion, congitive etc, this is not about all that goes into those decisions. Just wanted to share, had transrectal fusion biopsy 2 days ago. In/out of hospital in 3 hours; procedure took 20 minutes. Mild internal discomfort for 2-3 hours. No blood apparent. Urination same as pre-biopsy. Waiting for results. BTW: biopsy followed 7.2 PSA, 40% likelihood on 4K and Pirads 3 MRI; 75 yo
