Dr.Guttler’s comments: To prevent this serious complication, make sure you absolutely need the surgery. A total of 1000 RLNs from 574 patients were assessed in this review. Nearly one-quarter (23%) of the RLNs deviated from their anticipated anatomical course, 30% of RLNs were fixed, splayed, or entrapped at the level of the capsule of the thyroid. Entrapment at the ligament of Berry was observed in 41% of nerves. Traction injury was the most common cause of RLN injury (63%), especially at the site of the ligament of Berry. Thyroid (1) 2020 Tuttle RM et al Dr.Guttler’s comments: Study looked at 943 lobectomy cases matched with 943 patients with a total thyroidectomy for outcomes. At ten years there was no difference what surgery they had. Previous article by Tufano in the same Thyroid Journal found thyroid radiofrequency RFA was as good as surgery for micropapillary cancers, with less pain, no scar, loss of work, less expensive, and social and emotional outcomes were better. What does that say about the state of thyroid surgery in 2020? Less surgery is more. Lobe is as good as a total, and even better no surgery ..read more

Thyroid 32(1) 2020 Tufano RP et al “The year of technical advances in treating nodules and cancer” Dr.Guttler’s comments on this special article on thyroid advances. The number of surgeries for benign nodules has shown a dramatic drop since the 1970’s due to thyroid needle biopsies, thyroid ultrasound, molecular markers, ethanol ablation, and in 2018 the FDA approval of radiofrequency ablation for soft tissue use including thyroid nodules. RFA has now become the treatment of choice of benign thyroid nodules. “RFA has faster recovery times, less pain, higher social and psychological well being ..read more

Why do Functional medicine physicians order this test when it has no clinical value in the diagnosis and treatment of thyroid disease? Thyroid May 2018 Lopresti J.L. et al DR.Guttler’s comments: Article from USC thyroid Lab on orders of over 400,000 thyroid tests. 400,000 orders for thyroid tests. 90,000 orders for rT3. Only 20% of physicians ordered rT3 but only once or twice. 100/61,000 ( 0.1% ) providers ordered 30% of the rT3 tests. 60/100 were functional medicine providers. They say it is used to diagnose rT3 dominance and direct T3 replacement therapy. There is little evidence to suppor ..read more

Benign thyroid nodules class 11 are good candidates for Ablation. However, Class III will need more studies to be sure you don’t have a nodule with potential for thyroid cancer. A suspicious needle biopsy alone can lead to unnecessary thyroid surgery for a benign nodule. A suspicious molecular marker result for your class III cytology is still not a good way to avoid surgery for a benign nodule. Finally a PET/CT with low SUV function (<3 ) in the nodule is a good final way to avoid surgery with a benign nodule. Autonomous Functioning AFTN or Toxic Nodules. All Neck cysts can be treated wit ..read more
